Every employee is aware of the Employee Provident Fund (EPF) scheme, which works for the benefit of employees. An organisation with more than 20 employees must register their employees (with a monthly salary of more than Rs. 15,000) for the EPF. One of the important details to fill in while enrolling for EPF is the “Nomination”. Every EPF member must have a nominee on the account, and the details are submitted via EPF Form 2. In this article, let’s learn about the purpose of EPF Form 2 and how to fill the form. 

Purpose of EPF Form 2 

Every employee must submit a declaration and nomination under the Employees’ Provident Fund Scheme, 1952 and Employees’ Pension Scheme(EPS), 1995. In the case of the death of the EPF account holder, the nominated person will get access to the accumulated funds. You can fill and submit Form 2 at the time of EPF enrollment. 

Details on EPF Form 2 

EPF Form 2 (revised) has four sections – General information, Part – A (EPF), Part – B (EPS) (Para–18) and Certificate by the employer. The details of each part are as follows: 


General information

The general information of the EPF Form 2 contains basic details of the employees as listed below:

  • Name of the member
  • Father’s/Husband’s name
  • Date of birth
  • Gender
  • Marital status
  • Account number
  • Temporary and permanent address 
  • EPF and EPS joining date

Part A (EPF)

This part of Form 2 (revised) is about the nominee you like to add to your EPF account. The details you need to provide here are as follows: 

  1. Name of the Nominee(s)
  2. Address
  3. Nominee’s relationship with the member
  4. Date of birth
  5. Total amount or share of accumulations in Provident Funds to be paid to each nominee
  6. If the nominee is a minor, give the details of the guardian 
  7. Signature or thumb impression of the account holder (subscriber)

Part B (EPS) 

The EPF Form 2 contains the details about the Employees’ Pension Scheme. Note that 8.33% out of 12% of an employer’s EPF contribution is deposited in the EPS account of the member. The nominee will get access to the funds accumulated in the EPS. The details required are as follows: 

  1. Name of the family member
  2. Address of the family member
  3. Date of birth
  4. Relationship with the member
  5. Signature or thumb impression of the subscriber

In case of nomination for monthly widow pension (admissible under Para 16 2(a) (i) and (ii)), the member has to give detail such as:

  1. Name and address of the nominee
  2. Date of birth
  3. Relationship with member

Certificate by the employer

The employer needs to mention certain details as follows: 

  1. Name of the employee
  2. Authorised officer’s signature
  3. Designation of the officer
  4. Date
  5. Place
  6. Name and address of the organisation and stamp

Steps to fill EPF Form 2 online

An existing UAN member can submit the nomination form online by following the steps below:

  • Log in to the EPF member portal using your UAN and password.
  • Go to the “Manage” section and select the “e-Nomination” option.
  • Give the “Permanent Address” and “Current Address” and click on “Save”.
  • Select whether you have a family or not.
  • Enter your family details such as name, date of birth, gender, relation, address, guardian (in case of minors), Aadhaar number and click on “Save Family Details”.
  • Click on “Add Row” if you want to add more nominees.
  • Select the nominees added and their share on your EPF and save the details.
  • Follow the same steps for the EPS section for availing benefits of the pension scheme.
  • Now go to the “Manage” section and select the “e-Nomination” option to e-sign and complete the process.
  • To verify the details given, enter your Aadhaar number and click on “Get OTP”.
  • Enter the OTP you got on your registered mobile number. Now your nomination details are successfully registered with EPFO. 

Important points to note 

There are certain important points to remember while you are adding your nominees:

  • You are allowed to change their nomination whenever you want to do so.
  • Your nomination should be in favour of one or more member(s) of the family.
  • The subscriber is allowed to nominate anyone if they don’t have a family. 
  • The subscriber cannot add anyone out of the family as a nominee if they have a family. 
  • You can add more than one nominee to your EPF account. 
  • Once the subscriber gets married, they need to file a new nomination again.

1. Can I file an EPF nomination offline?

Yes. Fill out the revised EPF Form 2 and visit the nearest EPFO office with the identity and professional documents to register the nominees offline. 

2. Is it mandatory to have UAN registration to update nominee details online? 

Yes. You can activate your UAN account anytime on the EPF portal and complete your nominee declaration.
